Spay/Neuter Contract

By signing this contract, I agree to have our Mini-Labradoodle, purchased on this date, spayed/neutered between 4 to 5 months of the date of birth, which is ______________. Upon receipt of the spay/neuter certificate from a licensed veterinarian, breeder will immediately send to you the pedigrees of both parents.

I also guarantee that this puppy will not be bred, prior to neutering,  because of the risks involved, which the breeder has explained to me/us in detail, namely that the mother of this puppy is approximately 70 lbs. and the father is approximately 5 lbs.

This puppy is being sold on a spay/neuter contract. The new owner agrees to spay/neuter this dog and submit written proof of Spay/Neuter from a licensed Veterinarian to Labradoodles of Oregon, at the address below, prior to ______________2009. Failure to do so will allow the Seller/Breeder of this puppy, or its appointed officer, to reclaim immediate possession of the puppy and the purchase price will be forfeited. This puppy is being sold as a companion, not as a brood bitch or a stud dog.

I have read and fully understand this spay/neuter contract and will abide by these conditions.
